| To order your wristband(s) online: Step 1: Fill out the form (all fields required) below and
click 'submit form'. Step 2: Click on the 'pay now' button beneath the form. In the 'amount due' box add your total from the first form in the 'pay amount'
box. Proceed as Paypal directs. Your wristbands will be mailed to you within one business day of receipt of your order. | To order your wristband(s) via USPS: Send your name, address (include city, state, zip), telephone number and number
of wristbands you wish to purchase, along with a check* or money order to: Project Nevaeh Wristbands, ATTN: Donna Lindemann, 14 Neversink Avenue, Port Jervis, NY 12771 Your wristband(s) will be sent to you (via postal mail) within two business days of receipt of your order. Thank you! |
